Although newly grafted fat can survive several decades, the native fat of the face is programmed to shrink over time. For example, neck skin, which is qualitatively different from facial skin, can look windswept if incorrectly transplanted. Additionally, poor quality facial skin with deeply etched lines can create an appearance similar to this. The natural vertical lines in the skin can become more horizontal, resembling "kitten whiskers," requiring further attention through laser resurfacing. Another common issue is the widening of the mouth, a problem that can be exacerbated by uneven grafting techniques.

When done skillfully, the combination of a facelift and fat grafting can greatly reduce a person's perceived age by a generation. While some have criticized fat grafting, my extensive experience and long-term perspective have shown that a well-executed fat grafting procedure can provide long-lasting benefits.
